RE: URGENT COVID-19 VACCINE MANDATE EMERGENCY RULE

On August 26, 2021, the New York Public Health and Health Planning Council (PHHPC) and the New York State Department of Health (NYSDOH) approved an emergency mandate for all health care workers to be vaccinated against COVID-19 in the state of New York. This includes all Home Care direct caregivers such as RNs, LPNs, HHAs and PCAs. Therefore, you must have already received at least the first dose of the vaccine of your choice and submit a copy of your vaccination card to our office by October 7, 2021 or you will no longer be employed by the agency as of October 8, 2021.

There is NO RELIGIOUS EXEMPTION for receiving the vaccine allowed by PHHPC or NYSDOH. You must take the vaccine to continue working as a caregiver with any New York licensed home care agency. Only Medical Exemptions are acceptable. If a licensed physician or certified nurse practitioner certifies that immunization with COVID-19 vaccine is detrimental to your health, based upon a pre-existing health condition, you will be exempt until the immunization is found to no longer be detrimental to your health.

PLEASE NOTE THAT ALL MEDICAL EXEMPTION DOCUMENTS SUBMITTED REGARDING THIS VACCINE WILL BE CHECKED FOR VALIDITY IMMEDIATELY UPON SUBMISSION.

We have signed up with “Flex for Checks” to allow our unvaccinated home care workers to receive $75 per shot to get vaccinated and $25 per shot thereafter. You may also receive an additional $25 for referrals!
